You can make a referral for support from The Advocacy People by using our online referral system or by filling in a pdf form and sending it to us. You can find self help information and referral forms in our Document Library.

It will be useful to have to hand:

 

  • Information about the person the referral is being made about, such as full name, date of birth, full address and contact details.

  • Details about the reason the referral is being made, such as the name of others involved, organisations and details about the issue needing advocacy support and important dates (such as meetings, when things happened, etc. relevant to the referral)

 

If you are making the referral on behalf of someone else as either a friend/relative or as a professional as part of your job, you must have the person you are making the referral abouts consent to do so.
