You can make a referral for support from The Advocacy People by using our online referral system or by filling in a pdf form and sending it to us. You can find self help information and referral form in our Document Library.

Community Advocacy Referral Form

This form will take approximately 10-15 minutes to complete. You will be asked detailed information about the client and case, please also have ready supporting documentation for upload.

It will be useful to have to hand:

 

  • Information about the person the referral is being made about, such as full name, date of birth, full address and contact details.

  • Details about the reason the referral is being made, such as the name of others involved, organisations and details about the issue needing advocacy support

 

If you are making the referral on behalf of someone else as either a friend/relative or as a professional as part of your job, you must have the person you are making the referral abouts consent to do so.
